TO REQUEST AN APPOINTMENT

Appointments can be made by calling us on (07) 3831 6888. Our dedicated team of reception staff are ready to assist you.

You may also email us at appointments@cityeye.com.au or send us an email via our website link on the Contact Us page. Please advise our staff of your details if the referral is urgent, and we will do our best to prioritize your appointment. Your referring practitioner can also contact us directly. Practice hours are Monday to Wednesday 8am – 4pm and Thursday to Friday 8am – 1pm.

PRIOR TO YOUR APPOINTMENT

If you are a new patient to City Eye Centre, you will receive a Welcome Letter from us detailing your appointment and what to expect on the day of your appointment. As a new patient, your previous medical and surgical history will assist with our treatment plan. You may wish to download and fill out the new patient data collection and consent form prior to the appointment and email it to us at appointments@cityeye.com.au. This will save you from filling out the form upon arrival at our practice.

Please ensure that you have a current referral from your referring GP, optometrist or medical specialist at the time of your consultation. The referral should be forwarded to us prior to your appointment. Medicare rebate would not be claimable without a current referral. Unless specified, referrals from your GP and optometrist are current for 12 months while a referral from a medical specialist remains current for 3 months.

Please advise if you require transport assistance and documentation or have any specific travel needs. We will do our best to assist. If you are a DVA patient, please call our rooms prior to your appointment to confirm that DVA transport has been arranged for you.

ON THE DAY OF YOUR APPOINTMENT

Please bring your Medicare card, private health fund details, list of current medications and eyedrops, list of allergies, and any other relevant medical details on the day of your appointment or you can email us the information prior to your consultation. As your eyes may be dilated with eyedrops at your consultation, we advise that an accompanying person comes with you to the appointment and that you arrange transport as you may be unable to drive for at least 3-4 hours. It would be helpful to bring a pair of sunglasses which would reduce problems with glare.
